Call the Health Care Access Program (HCAP) Hotline at (212) 503-6804 to be pre-screened and make an appointment to complete your application. HCAP will submit the application directly to the New York City Human Resources Administration (HRA) or Child Health Plus Plan. You and your family may qualify for the following programs regardless of resources such as bank accounts or property owned:

  • Medicaid: For children and adults with little or no income. Eligibility also is determined by family size and immigration status
  • Child Health Plus: For most children under 19 years old, regardless of their income or immigration status
  • Family Health Plus: For adults 19-64 who do not qualify for Medicaid. Eligibility is determined by family size, income and immigration status

Benefits covered by these programs include:

  • Regular checkups
  • In/outpatient hospital care
  • Prescription medication
  • Vaccination
  • Dental care
  • Emergency care
  • Mental health care
  • Vision, speech and hearing services

Documentation you will need to apply:

  • Proof of address (utility bill, lease or postmarked envelope)
  • Proof of ID/Immigration status (birth certificate plus photo ID, passport or U.S. Permanent Resident Card, also known as a "green card")
  • Social Security cards (if available)
  • Proof of income (pay stubs, unemployment, child support or self-employment) You do not need to be working to qualify.
