Medical Foster Care Social Worker

Job Responsibilities:

• Carry a caseload of 12-14 medically fragile and/or developmentally disabled children in foster care
• Engage birth parents to make assessments and provide referrals and interventions that will facilitate re1miflcation with th\lit children
• Work collaboratively with registered nurses to insure that children's medical, mental health, educational, and social needs are met
• Provide supportive services for foster and adoptive parents
• Participate in family court hearings, including giving testimony
• Participate in case conferences
• Field work and home visits are required
• Perform the basic accountability tasks related to the Job, which include compiling and maintaining all necessary case record documentation
• Perform other duties as needed


• MSW preferred
• Experience working with multi-problem and economically disadvantaged families
• Strong clinical skills, including the ability to engage birth parents with histories of trauma, substance abuse, mental illness and domestic violence; ability to identify client strengths.
• Experience worldng in a health care setting a plus
• Ability to work collahomtively and independently
• Excellent written and verbal communication skills
• Ability to work with respect for cultural diversity
• Bilingual (Spanish/English) required
• Driver's license preferred
